TO APPLY FOR FUNDING FOR A “PIT BULL” PROGRAM . . .

 

In making an application to Animal Farm Foundation, Inc. please include all of the following information, as it applies to your organization. If any of the requested information or documents is not included, please tell us why.

 

  1. The full name of your organization, mailing address, address of operating facility (if different from mailing address), web address, contact person, phone numbers, and email addresses.

 

  1.  A copy of your mission statement.

 

  1. Describe your principal activities. How long have you been in operation? If you have a “pit bull” adoption program, please describe your requirements for intake, selection, evaluation, housing, socialization/training and final disposition (adoption, transfer, euthanasia). Describe your requirements for potential adopters. Detail any other programs, policies and materials regarding “pit bulls.”

 

  1. Please describe the specific “Pit Bull” program for which you seek funding: the scope of the project, its goals, and the manner in which you will measure your success in meeting these goals. 

 

  1. Include a budget for the program for which you seek funding.  Do you have a multiple-year plan for the program? If so, how do you expect to sustain it? Is this program reproducible by other organizations? How?

 

  1. If applicable, a description of any operating facilities.  Include photographs.

 

  1. A copy of your 501(c) (3) letter. Have you ever applied for non-profit status and had your application denied? If yes please explain.

 

  1. Your approximate annual expense budget and a copy of your most recent federal tax return. A copy of your most recent certified audit statement.

 

  1. If not included elsewhere (e.g. on your tax return), list the names and compensation of all directors.

 

  1. Are you applying to other organizations for the same funding? If you are, please notify us immediately of any other funding you obtain for the program.
